In partnership with IndyCare Medical and Truentity Health, we are supporting our pharmacy patients with chronic conditions through Remote Patient Monitoring (RPM).

What We Offer

  • No Cost Blood Pressure Monitors and Blood Sugar Meters: We provide free devices and supplies, including test strips and batteries. Trouble with the device? We provide full support at no cost to you.
  • Daily Monitoring & Outreach: Your readings are automatically sent to the pharmacy for monitoring, allowing our care team to catch problems early and reach out to you.
  • Monthly Reports to your Primary Doctor: We help your doctor’s office make decisions by providing monthly reports and recommendations based on your readings at home.

How We Support Your Doctor’s Office

We act as an extension of your care team - monitoring your conditions between doctor visits for more complete care and accessibility.

Who is Eligible?

  • Medicare and Medicaid patients with a current diagnosis of hypertension or diabetes.
  • You must take regular readings to stay enrolled in the program - we can’t help you without them!
